                              I had opened a Life Insurance policy in your [LIC, Branch name] on [Date]. The policy number is [Policy Number] and the premium for the said policy is all cleared till date. I am very much happy and satisfied with the service of the LIC branch and friendly attitude towards the customer.

                              I would like to kindly inform you that I have changed my surname as I have got married recently. Therefore, I would kindly request you to update the necessary information of the policies under [old name] to [new name].Necessary documents are attached for the smooth functioning of the procedure.

                           I would be very much happy to oblige any other formalities required by LIC. I would personally visit the branch and provide other necessary documents as and when required.
