Subject: Rejoining of duty

Dear Sir/ Madam,

Most humbly, I would like to inform you that I could not attend my duty since [Date] as my brother, Mr XYZ had exposure to corona virus and consequently our home had been declared as containment zone by authority.

Unfortunately, I also got infected by corona virus and tested positive on [Date]. On subsequent testing on [Date], I was found to be negative for Rapid Antigen Test.

Hence, after spending a week under home isolation as advised by health authority, I would like to join duty from today.

I would be grateful if you look into the matter urgently.

This is for your kind information and necessary action.
