Survival benefit is a certain amount of financial benefit given to a policy holder by an insurance company provided the policy holder is alive during or upon completion of the policy period. Sometimes, it may happen that the policy holder may not get the survival benefit within stipulated time period due to some technical difficulties. If one does not receive one’s survival benefit due to some reason, it is required to bring the matter to the notice of the competent authority.                         With reference to the above mentioned policy number, I wish to bring to your notice that the due date of the survival benefit of the concerned policy was on [Date]. But with much disappointment, I would like to state that I have not yet received the survival benefit from your office.

                          Kindly, look into the matter urgently so that delivery of the receipt is made at my residence. I would be obliged if you can guide me for any other formalities regarding this matter.
